Managing life in today’s time poor environment can be a juggling act, especially if you have a health condition. If you have Total Extras cover, our Health Management Program benefit can assist by supporting certain treatment options recommended for you by your doctor.
What programs are covered?
We offer a benefit for the following medically prescribed treatments:
exercise classes conducted at a gym or by a personal trainer
weight loss programs
quit smoking programs
What are the eligibility requirements?
The program must be undertaken to treat a specific medical condition that’s been diagnosed by your medical practitioner. Examples of conditions are asthma, arthritis, obesity or weight gain, high blood pressure and musculoskeletal disorders.
Can I choose my provider?
Yes. In keeping with our Fund principles and government regulations, the program must be delivered by appropriately trained individuals registered with an accredited national association. Exercise physiologists and RACGP medical acupuncturists must have a current Medicare provider number.
How much is the benefit?
The benefit is 50% of the cost up to $200 per person per year with an overall limit of $400 for family policies.
When can I claim for my Health Management Program benefit?
You can claim the benefit once you have both received the prescribed treatment and paid for it. For example, if the Program involves gym classes and you’ve paid your gym membership in advance, you can only claim once the period to which the payment relates has been completed.
How do I claim?
You can submit your claim to us via the Doctors' Health Fund mobile app, email (to firstname.lastname@example.org), fax or post. Claims through the mobile app need to include a photo of your completed Health Management Approval form.
